Interacts as an interdisciplinary team member in the assessment, planning, and implementation of treatment plans for patients in one or more specialized areas of medical social work. Complies with all appropriate safety and infection control standards. Participates in quality assessment and continuous quality improvement activities. Performs all job duties and responsibilities in a courteous and customer-focused manner according to the Hurley Family Standards of Behavior. Works under the direction of a director/manager or designee who reviews work for conformance with Medical Center policies and procedures. Clinical supervision will be provided by the Senior Social Work Case Manager or other Licensed Masters Social Worker. May supervise other professional social work staff such as Bachelor's prepared social workers, Master's and Bachelor's social work interns, and volunteer staff.

1. Completes psychosocial/environmental assessments on patients and patient families in order to obtain history data to evaluate level of psychosocial functioning.


2. Provides documentation, summaries, and evaluations of patient histories, psychosocial functioning, needs, and progress to appropriate parties in timely manner.


3. Provides counseling/therapy to patient and patient families in order to assist in resolution of psychosocial reactions to hospitalization, illness, and/or crises.


4. Facilities referrals to appropriate Medical Center personnel and community support agencies.


5. Assists patient and family in developing after-care plans in conjunction with other Medical Center disciplines.


6. Assists in implementation of legal processes. Acts as liaison among Medical Center, patients, families, courts, staff, and other community support agencies.


7. Performs crisis intervention and safety assurances as needed.


8. Compiles statistical reports and other data for departmental reports on assigned cases.


9. Maintains expertise in field through appropriate educational/organizational activities. Conducts in-service education programs related to medical social work.


10. May represent department within Medical Center and/or community via committee membership, speaker bureaus, professional memberships, and advisory boards.


11. Coordinates related projects and assignments as required.


12. Demonstrates effective judgment and ability to understand, react effectively and treat (if appropriate) unique needs of patient age groups served.
