Job Description

Processes in-person, telephone, and written requests for the release of medical information; initiates, completes, and submits certificates of live births and reports of birth defects to Bureau of Vital Statistics. Completes acknowledgements of paternity for Probate Court. Participates in quality assessment and continuous quality improvement activities. Complies with all appropriate safety and infection control standards. Performs all job duties and responsibilities in a courteous and customer-focused manner according to the Hurley Family Standards of Behavior. Works under the general supervision of the departmental director or designee who assigns and reviews work for completeness and accuracy.

Responsibilities

  1. Processes in-person, telephone, and written requests for the release of medical information in accordance with established policies and procedures.

  2. Reviews authorizations for the release of medical information to ensure legality.

  3. Logs requests for medical information in appropriate location; maintains clerical records of requests; prepares reports of Correspondence Unit activity from these reports.

  4. Handles telephone and in-person public contacts from physicians, attorneys, insurance company representatives, Vital Statistics representatives, patients, other health care institutions, law enforcement agencies, schools, places of employment, governmental agencies, social agencies, and third party payors; provides information and explains policies and procedures to requesting parties.

  5. Accepts subpoena duces tecum relative to medical center medical records; collects associated fees; processes in accordance with medical center policies and procedures and statutory provisions.

  6. Screens all patient records prior to releasing medical information to determine treatment related to substance abuse, psychiatric treatment and communicable diseases; follows specific release of information guidelines associated with each as required.

  7. Interviews mothers in order to complete certificates of live births; ensures submission on a timely basis to the Bureau of Vital Statistics.

  8. Completes forms for logged statistics regarding Birth Defects Registry and submits to the Bureau of Vital Statistics.

  9. Maintains copy machines and reader-printers.

Qualifications

  • Education equivalent to completion of high school.
  • Six (6) months of experience in responsible clerical work.
  • Continuation in this classification requires that the employee, upon completion of six (6) months of training/orientation, has successfully met the quality and quantity standards of the job.
  • Ability to type accurately onto forms.
  • Ability to deal with patients, physicians, Medical Center staff, other professionals and the general public tactfully and courteously.
  • Ability to communicate effectively in both oral and written format.
  • Ability to make rapid and accurate arithmetic calculations and tabulations.
  • Ability to maintain clerical records and to prepare required activity reports from such records.
