Job Description
Provides technical support in obtaining, verifying for accuracy, and recording medication histories from patients and/or their designated family/caregivers upon presentation to the medical center. Records information accurately and efficiently. Participates in quality assessment and continuous quality improvement activities. Ensures compliance with all appropriate safety and infection control standards. Performs all job duties and responsibilities in a courteous and customer-focused manner according to the Hurley Family Standards of Behavior and Code of Conduct.Works under the supervision of a Pharmacy manager or designee, who reviews work for proper completion and conformance with established procedures.
Responsibilities
- Retrieves medication history from past encounters using the electronic medical record, if available, prior to meeting with the patient and/or family/caregiver.
- Interviews patients and/or their designated family/caregivers as soon as possible after presenting to the organization (includes Emergency Department, inpatient, and outpatient areas).
- Obtains and records in the electronic medical record a current list of medications to include: prescription medications, over-the-counter (non-prescription) medications, vitamin and nutritional supplements, as well as the source(s) for this information.
- Recording should include the following: name/location of the patient's ambulatory pharmacy, medication name, dosage form, strength, dose, day/time of most recent dose, and source(s) of the information being recorded.
- Attempts to clarify missing or suspect data by consulting physicians, pharmacies, or other medical facilities.
- Obtains, records, and verifies in the electronic medical record, patient allergy information including the nature of the allergic reaction when necessary.
- Consults with a pharmacist in all cases of unresolved issues related to a patient's medication and allergy history.
- Makes a concerted effort to return to patients not available for interview when identified.
- Records and maintains daily activity data.
- Works as an integral part of the patient's healthcare team. Communicates and interacts with team members as appropriate.
- Always introduces self and explains purpose of visit in a consistent customer-focused manner.
- Performs other related duties as required. Utilizes new improvements and/or technology that relate to job assignment.
