RN - Case Manager - 40 hours - Days

GENERAL SUMMARY:
The Case Manager-RN coordinates the care and service of patient populations. The key responsibilities are to partner with the healthcare team to ensure all aspects of the patient’s needs, clinical, psychosocial, and financial are adequately addressed in the transition of care plan and to manage the patient’s timely progression of care and safe transition to the next most appropriate level of care. Demonstrates positive interpersonal communication with patients, families, visitors, and co-workers in accordance with organization and departmental policies and procedures.
EDUCATION/EXPERIENCE REQUIRED:
- Bachelor’s degree in nursing.
- Three (3) years of recent acute care nursing experience.

OR Associate’s degree in nursing and two (2) years of current acute case management experience. Enrollment in an accredited BSN program within three (3) years of hire, and completion of the BSN program within five (5) years of hire is required.

- Certification in Case Management (CCM) by the Commission for Case Management Certification or Accredited Case Manager (ACM) by the American Case Management Association required within one (1) year of hire.
