The Case Management Social Worker coordinates the overall interdisciplinary plan of care for the patient, from admission to discharge. Responsibilities:
- Creates plan for care across the continuum, integrating patient/family preferences and values
- Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs
- Advocates for resources and removal of barriers
- Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan
- Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care
EDUCATION/EXPERIENCE REQUIRED
- Social Worker credentialed from the Michigan Board of Social Work obtained prior to hire date or job transfer date required, any level is accepted
- High school diploma/GED with 2 years of experience, or associate's degree, or technical degree required
- Needs discharge planning experience
- Medical Terminology preferred
- Willing to learn Medical Social Work
- Behavioral health with resources and placement helpful
